Blood Antibody Testing is shown to be of limited value in H. pylori management

Blood Antibody Testing is currently the most widely used non-endoscopic test for H. pylori assessment.

However, the 2007 guidelines for Helicobacter pylori management by the American College of Gastroenterology (ACG) state that the Blood Antibody Test for Helicobacter pylori detection has significant clinical and diagnostic limitations. The guidelines recommend Urea Breath Testing (UBT) over Blood Testing in most cases. Consider the following:

In low-prevalence populations, as in the United States, Blood Testing “should be avoided altogether.” If used, positive results should be confirmed by UBT.

Regarding Blood Testing, “a positive blood test is considered no better than a coin toss in predicting active infection.”

When testing to determine post-treatment eradication, Blood Testing has only limited benefit, since test results can appear positive for years following successful treatment.

UBT “is the most reliable non-endoscopic test to document eradication of active infection.”
Numerous recent studies compared different testing modalities including Urea Breath Tests, serology tests (serum IgG and IgA antibodies to H. pylori), and endoscopic biopsy. With positive predictive values of more than 95 percent for detecting H. pylori in all patients and more than 97 percent in patients with active peptic ulcers, the studies demonstrated that the Urea Breath Test is an accurate noninvasive test for detecting H. pylori infection.

Researchers confirm non-O ABO blood groups and Helicobacter pylori are linked to pancreatic cancer.
Researchers found that the presence of H. pylori in people with non-O blood triples the risk of their developing pancreatic cancer. In the U.S., Pancreatic cancer affects more than 1% of the population and is the 4th highest cause of death due to cancer. Prior to this study, known risk factors explained only about a third of cases; the new findings will pave the way for more exploration into the etiology of the disease.

New study: Eradicating Helicobacter pylori may prevent coronary disease, metabolic syndrome.
The close association between Helicobacter pylori and insulin resistance has been well established; a new study, comprised of 159 patients, aimed to look at the effects of eradicating the bacteria on insulin resistance, serum lipids, and inflammation. The single-center study found there was a positive effect of H. pylori eradication, suggesting that eradicating the bacteria may prevent coronary disease.

Japan: Helicobacter pylori infection management guidelines revised
The relationship of H. pylori and gastric cancer has become more widely established in the last few years which had prompted the Japanese Society for Helicobacter Research to meet and revise its guidelines. H. pylori eradication therapy achieved a Grade A recommendation, being useful for the treatment of gastric or duodenal ulcer, for the treatment and prevention of H. pylori-associated diseases such as gastric cancer, and for inhibiting the spread of H. pylori infection.
